The Community Rebirth Foundation. advances equitable health outcomes and strengthens community wellness through education, advocacy, and culturally grounded support. We center the voices and lived experiences of Black and Indigenous families.
We focus on the full continuum of care. From reproductive and maternal health to cancer survivorship and holistic healing, we bridge the gaps that too often leave our communities unseen. Through training programs, storytelling, and collaborative partnerships, we empower individuals to reclaim their health, their heritage, and their wholeness.

So many people are carrying heavy battles in silence, trying to be strong, trying to push through, trying not to “burden” anyone with what they’re going through. But the truth is simple:
No one heals well alone.
That’s why the Community Rebirth Spiritual Mentorship Program is built on the power of peer connection.
Rooted in Black faith traditions, community care, and trauma-informed support, we offer one-on-one prayer, spiritual counseling, and ongoing mentorship for those navigating:
- cancer or chronic illness
- postpartum transitions
- grief and loss
- mental and emotional heaviness
- spiritual fatigue or disconnection
- major life changes or identity shifts
Whether your relationship with faith is strong, complicated, brand-new, or rebuilding, this is a place where you can breathe, be held, and reconnect with hope.
